Man run over while overhauling bus at Chicago Heights junkyard
Chicago, IL — A man died after being run over by a bus he was overhauling Friday afternoon at a junkyard in south suburban Chicago Heights. Officers responded at 2:17 p.m. to the incident at Wilkins Rebuilders Supply, 171 E. 12th St., according to Chicago Heights police and the Cook County Medical Examiner’s Office. When they arrived, officers learned that an employee driving a front end loader started moving the bus without realizing that 18-year-old Arlen J. Bradley was working underneath it, police said. The loader’s operator then moved the bus forward, causing the rear wheels to run Bradley over.
Sanitation worker dies working at Arkansas school
CALICO ROCK, Ark.—A sanitation worker has died while working at a northern Arkansas elementary school. The Baxter Bulletin reports that 48-year-old Steve Allen died Wednesday after a dumpster fell on him as he emptied it into his garbage truck. A Calico Rock Public School employee found the Methvin Sanitation employee unconscious at the campus.
Dumpster falls and crushes man to death in East St. Louis, officials say
East St. Louis, MO – A man finishing painting a large waste container was killed when he was crushed by the falling container on Friday morning at a business in East St. Louis. A St. Clair County coroner’s investigator was called to Wastequip, 2701 Converse Ave., on Friday afternoon. St. Clair County Coroner Calvin Dye Sr. identified the deceased man as Kenneth J. Woltering, of Carlyle.
CONSTRUCTION WORKER DIES AT HEIGHTS SCHOOL
Potomac, MD — A construction worker died after an accident at the Heights School in Potomac on Thursday. Montgomery County Police have identified John Thomas Moran Jr., 27, from North East, Md., as the construction worker who died while working on a construction project for the school. Fire and rescue services responded to a call at the Heights School on Seven Locks Road at around 8:43 a.m. Thursday for a report of a construction accident. According to police, the construction crew had been working on moving a portable building outside the school, when the moving building struck Morgan, who died from his injuries at the scene.
Truck driver killed in Franklin County crash
FRANKLIN COUNTY, MO — A man was killed Friday morning when his truck went off a highway here and crashed into a barn and house. Michael V. Ley, 57, of Washington, Mo., was southbound on Highway 185 about 10:25 a.m. when his truck traveled off the right side of the roadway south of Liberty School Road, according to a Missouri Highway Patrol crash report. Ley’s 1990 Freightliner FL70 truck skidded and he pulled it back onto the highway, but the truck went off the left side of the roadway. The truck then struck a barn before it crashed into a house, police said. Ley was pronounced dead at the scene. He had been wearing a seat belt.
Man killed in industrial accident
Beaumont, TX — A man has died after being struck in the head with logs in an industrial accident Wednesday, according to information from the Liberty County Sheriff’s Office. At approximately 12 p.m. officials responded to a call about a man who was hit in the back of the head at South Texas Logging & Transport Company, LCSO said. Matthew Aaron Sellers, 33, is believed to have been in the blind spot of the loading vehicle carrying logs when he was struck.

Boyertown, PA — A Boyertown-area man was killed in an industrial accident at a Robeson Township quarry, authorities said Thursday morning. Brent Cosner, 29, of Colebrookdale Township was pronounced dead at 1:15 a.m. at his workplace at Birdsboro Materials, 1267 Haycreek Rd., by Berks County Deputy Coroner Kurt E. Katzenmoyer. Officials said Cosner got caught in a conveyor belt at the quarry.
NY officer dies of 9/11-related illness
NEW ROCHELLE, NY — A 20-year veteran officer of the New Rochelle Police Department died Aug. 16 from cancer developed from assisting search and rescue efforts after the Sept. 11 terrorist attacks. Officer Katheleen O’Connor-Funigiello is survived by her husband, three step-children and five siblings, ODMP reports. Seventy-two officers from eight local, state and federal agencies were killed on Sept. 11, 2001 when al Qaeda terrorists crashed four hijacked planes into the World Trade Center towers in New York City, the Pentagon in Arlington, Virginia and a field near Shanksville, Pennsylvania. Heavy air contamination resulting from the attacks at Ground Zero led to the illness and deaths of several first responders in the years following the incident.

GREENBURGH, NY – A moving company employee is dead after being pinned under a tractor-trailer in the town of Greenburgh. Police say the accident happened on Spencer Court near Shaw Lane on Wednesday morning. The victim is a 36-year-old man who worked for the “Reliable Van and Storage” company in New Jersey. Police say the driver of the moving truck accidentally backed up while the victim was trying to fix the wheels.

HARBORCREEK, PA – Authorities say a man was driving drunk when he struck and killed a Wisconsin woman who was working at a construction site in western Pennsylvania. Jacqueline Ohly was helping to install cable guide rails along Interstate 90 in Harborcreek when she was struck around 7:30 a.m. Tuesday. The 26-year-old Stevens Point resident was pronounced dead at the scene a short time later.
Yorktown man dies after industrial accident in Hampton
HAMPTON, Va. (WAVY) — A Yorktown man died after a work-related accident in Hampton on Wednesday. According to the Hampton Police Division, the accident happened just before 10:40 a.m. in the 2500 block of McMenamin Street. Officers found the 60-year-old at the scene suffering from life-threatening injuries. He was taken to the hospital, where he later died.
Deliveryman dies after fall in Barre
BARRE, Vt. — Police say a deliveryman died after falling down a staircase Wednesday afternoon in Barre.It happened around 2:45 at Simply Subs restaurant on Main Street. Police say Louis Mason, 63, was making a delivery for the Capital Candy company when he apparently fell down a basement flight of stairs in the back of the restaurant. Authorities say the Montpelier man was found unresponsive at the bottom of the stairs with unknown injuries. Attempts to revive him were unsuccessful and he was pronounced dead at the scene. Police say his death appears to be an accident. An autopsy is pending.
2nd Death From Accident Along West Virginia Turnpike
PRINCETON, W.Va. (AP) — State Police say a second person has died in a truck accident on the West Virginia Turnpike. The Bluefield Daily Telegraph reports State Police say in a news release that 21-year-old West Virginia Parkways Authority employee Richard Lambert died Saturday at a hospital in Roanoke, Virginia. State Police said a tractor-trailer hit two Parkways Authority vehicles on the side of the road and the three employees standing next to them Thursday along Interstate 77 in Mercer County. Also killed was 32-year-old Parkways employee Nathan Thompson. Both Thompson and Lambert were from the Kegley area. Nineteen-year-old Ethan Kestner of the Princeton area was injured.
Workplace Accident Resulted in Death of Frostproof Man
Frostproof, FL — On Monday, August 20, 2018 at 9:22 a.m., deputies from the Polk County Sheriff’s Office, along with Frostproof Fire and Polk EMS, responded to a fatal workplace accident at David Keen’s Auto Repair at 101 Scenic Highway North, in Frostproof. 53-year-old Brian Jones, of 394 6th Street West, Frostproof, was pronounced deceased at the scene by paramedics. According to the preliminary investigation, a truck which was being worked on by Jones, began to roll off of a mechanical lift. Jones tried to stop the truck from rolling, and it fell on top of him.
Michael McDonald, retired FDNY from Farmingville who worked at Ground Zero, dies
New York, NY — On Sept. 11, 2001, FDNY firefighter Michael McDonald was watching TV from his Farmingville home as the first plane struck the World Trade Center. He left home that day, heading straight to Ground Zero via the Long Island Expressway, which was closed to all traffic except emergency responders. “He was there when the second tower fell,” Diana McDonald said. “He worked long shifts in the forthcoming days, weeks and months at Ground Zero.” On Aug. 11, Michael McDonald died of lung cancer. He was 64. He is the 181st member of the FDNY to die from a 9/11-related illness, officials said. He retired in May 2012 after 28 years of service. He spent his entire career at Ladder 128 in Queens.
Worker dies after being hit by 2,000-pound steel beam near downtown Kansas City
Kansas City, MO — A spokesperson for the Occupational Safety and Health Administration says a worker has died after being hit by a steel beam Monday in Kansas City. The incident happened about 4:30 p.m. at an industrial site east of downtown. According to the spokesperson, the man was working for Continental Steel Fabrication when he was hit by a 2,000-pound steel beam and killed. Family members have identified the man as Rick Ham. The spokesperson says OSHA has opened an investigation into the incident to determine if the company was following all regulations at the time of the incident.
Employee opens fire at Texas food warehouse, killing manager, police say
Missouri City, TX – A woman opened fire at a large food warehouse in Texas early Monday, killing a manager and injuring a fellow employee, authorities said. The alleged shooter, an employee of the warehouse, also died after the incident and subsequent shootout at the Ben E. Keith Foods building in Missouri City, about 20 miles outside of Houston, police said. Employee Kristine Peralez allegedly shot the two victims around 2 a.m. when about 20 to 25 employees were in the facility, according to the Missouri City Police. Warehouse manager Francisco Reyes was shot and died at the scene, police said, and employee Fredencio Janas was hospitalized to be treated for a gunshot wound to his leg.
Firefighter Killed In Mendocino Complex Died After Fire Retardant Drop
SACRAMENTO, CA — A preliminary report says a firefighter died last week from falling tree debris after a retardant drop on California’s largest-ever wildfire. The “blue sheet” summary report by California fire officials says Battalion Chief Matthew Burchett was struck by falling tree debris on Aug. 13 at the Mendocino Complex Fire after a large air tanker completed a drop. Three other firefighters had minor injuries.
Driver pinned, killed by own rig after slipping off rolling truck’s running board

Conway, AR — A truck service worker died after he was hit by a vehicle on Interstate 40 in Conway around 2 a.m. Aug. 12, according to an Arkansas State Police report. Kenneth Weldon-Trase Ellis, 33, of Leslie had just finished repairing a flat tire on a tractor-trailer that was parked on the shoulder of the 127 mile-marker entrance ramp when he was hit by a 1986 Nissan, authorities said.
Arab American police officer killed in hit-and-run in Detroit
DETROIT, MI — A Detroit police officer died Wednesday after being struck by a hit-and-run driver on Saturday Aug. 4 on the west side of the city. At a news conference at the Detroit Public Safety Headquarters, Police Chief James Craig said that officer Fadi Shukur would “always be remembered for his magnetic smile.” Mayor Mike Duggan and other city and police officials were in attendance. “Everyone’s heart is broken in the city for a good man, a newlywed,” Duggan said. “Our prayers are with his family.” Shukur, 30, was struck while he was walking to his car after assisting other Detroit Police officers responding to a crowd control situation in the area of the 18000 West McNichols. He suffered a brain injury and was hospitalized in critical condition until he died Wednesday.

Worker dies after falling into vat of oil near Disney World
Orlando, FL — A man died after falling into a vat of oil at a plant Disney World uses to recycle food waste, according to the Washington Post. The incident happened shortly after midnight Tuesday at a facility near Disney World. According to the Post, two men were emptying oil and grease into the vat when a worker, later identified as 61-year-old John Korody slipped and fell into the vat. Korody’s co-worker tried to pull him out, but the fumes from the oil and grease overwhelmed both of the men. A spokesperson for Walt Disney World confirmed Korody wasn’t an employee of the resort, but of Harvest Power, the company that converts the food waste into renewable energy.

Quincy, WA — Ramiro Zavala, orchard manager for L&B Enterprises was making rounds of orchard on an ATV after checking on night time spray operation when he struck the end of a row of apple bins. He was propelled through the air and suffered fatal head trauma and broken neck. Zavala’s body was found in the morning by another employee heading to work. Plumber, Father of 2, Dies in Philadelphia Trench Collapse
PHILADELPHIA, PA –A worker has died after being trapped in a trench in Philadelphia’s East Oak Lane neighborhood Thursday morning. Family members who prayed as they waited hours for word about the man identified him as Anthony Smith, a master plumber and father of two. “He’s a great family man, he has two beautiful children, a great wife,” family member Kevin Blake told NBC10’s Aaron Baskerville. “He’s a hard working guy bro, he helps everybody.” Two other workers were nearby and were not hurt. But Smith, who was in the trench when it collapsed, was trapped for most of the day. Despite lengthy rescue efforts by about 100 responders, he could not be saved. Rescuers worked for about five hours to locate Smith, whose body was found about 15 feet below ground level among concrete, wiring and pipes. His body was finally recovered after nearly nine hours of digging.

Port Arthur, TX — Police have released the name of a man who died after being exposed to a chemical inside a tank at Tubal-Cain Marine Services on Friday. The deceased is identified as Tyler Winfree, 21, of Bridge City. Police were called around 1:34 p.m. to the facility at 8700 Old Ferry Road near the Veteran’s Memorial Bridge in reference to a man down, according to police. East Orange, NJ — A 55-year-old grocery store employee was killed Thursday morning in East Orange, authorities said. Tarlok Singh, who worked at the Park Deli & Grocery on North Park Street, was pronounced dead at the deli on Thursday morning, according to a statement released from the Essex County Prosecutor’s Office. He was killed at about 7:30 a.m., authorities said. Singh’s death was ruled a homicide by the medical examiner, the statement said. A cousin, Karnail Singh, told ABC7 that Tarlok Singh was stabbed in the chest.
Texas Gas Pipeline Explosions Kill 1 Worker, Injure 4 Others
HOUSTON, TX – One worker died after a fire and series of pipeline explosions two days ago in Midland County, Texas, officials said on Friday. The name and age of the worker who died was not provided. He succumbed to his injuries on Friday. The explosions occurred late Wednesday morning, when workers from two pipeline companies were investigating a underground gas leak. Two firefighters on the scene were also injured during the explosion. Kinder Morgan Inc and Navitas Midstream Partners both said at least one employee was injured. The accident occurred in the Permian Basin, where oil and gas activity has surged in recent years.
Employee killed in gruesome accident at South Valley processing plant
Lemoore, CA — A tomato plant employee died after he was run over by a tomato truck Thursday morning in Lemoore. Just after 5:30 a.m., officers received a report of a traffic collision at the Olam Tomato Processing plant in the 1100 block of South 19th Avenue, according to Lemoore Police Department. An investigation revealed an employee driving the truck and trailer was having mechanical problems pulling out of a loading dock. The victim, Randall Phillips, 59, of Lemoore, attempted to help the driver by releasing a valve that was underneath the trailer. The driver, not knowing Phillips was underneath, began driving the truck forward, causing a trailer to strike him, police said.

LOVE COUNTY, Okla. – An electrical worker was killed in an accident in Love County earlier this week. It happened near the intersection of Highway 32 and Highway 96 in western Love County around 1 p.m. Thursday. According to KXII, 45-year-old Ronald Moore, an employee of Red River Valley Rural Electric Association, was killed “after coming in contact with an energized power line while working to maintain electric infrastructure.”

Redding, CA — In the history of California wildfires there has never been anything like it: A churning tornado filled with fire, the size of three football fields. An official report describes in chilling detail the intensity of the rare fire phenomenon and how quickly it took the life of Redding firefighter Jeremy Stoke, who was enveloped in seconds as he tried to evacuate residents on July 26. . The smoke-and-fire tornado was about 300 metres wide at its base and shot approximately 12 kilometres into the sky, according to the report by the California Department of Forestry and Fire Protection. It reached speeds of up to 265kph, with temperatures that likely exceeded 1,480 degrees Celsius. Stoke was one of eight people killed since the blaze started. The 37-year-old fire inspector was driving his pickup truck down a Redding road, working on evacuating people from the larger blaze, when he radioed out a “mayday” call, according to the report. Stoke said he, “needed a water drop and was getting burned over”, the report said. Then Stoke’s transmissions abruptly stopped. His remains were not found until the next day.
Seekonk truck driver killed in Kingston rig crash
KINGSTON, MA — A 49-year-old Tractor Trailer truck driver from Seekonk was killed Friday morning when the rig he was driving ran off the side of Route 44 in Kingston and crashed. The crash occurred about 6:25 a.m. State police said the driver was taken to Beth Israel Hospital in Plymouth, where he was pronounced dead. Preliminary information indicates the driver may have suffered some sort of medical issue just before the crash. The truck was traveling east when it ran off the right side of the highway.
One flagger killed, 2nd injured in work-zone crashes
Bend, OR – A 76-year-old Milton-Freewater man faces manslaughter, DUII, hit-and-run and other charges in a crash Tuesday night in a northeast Oregon repaving project construction zone that killed a highway flagger, Oregon State Police said Wednesday. Leman Bledsoe was arrested in the crash that occurred around 8:40 p.m. Tuesday on state Highway 11 at milepost 16 near Athena, OSP Captain Tim Fox said. A preliminary investigation indicates the construction zone flagger, Tyresa Monaghan, 49, of Kennewick, Wash., had a vehicle stopped when Bledsoe’s silver 2005 Chrysler minivan side-swiped the stopped vehicle and struck Monaghan. She sustained fatal injuries and died at the scene, Fox said.
Brick Silo Falls, Kills Worker in Sedalia
SEDALIA, Mo – A bricklayer has died while working on a job site Wednesday in west-central Missouri. A U.S. Department of Labor spokesperson tells Missourinet preliminary information indicates that workers were building a brick silo when the silo fell and struck the worker, eventually leading to the employee’s death. The U.S. Department of Labor’s Occupational Safety and Health Administration (OSHA) has opened an investigation into the death of the employee of Independence-based Bonilla Masonry LLC. The general contractor at the site is Springfield-based Morelock.
Andover golf course worker dies following pillar collapse
ANDOVER, Kan. (AP/KWCH) — An employee at an Andover country club has died days after a brick pillar fell on him. Andover police say Jeff Williams died Wednesday from injuries he suffered on Sunday at the Terradyne Country Club. Williams was an assistant superintendent at the golf course. Chief Chad Russell says the pillar fell on the 27-year-old man while maintenance crews were digging trying to find a water leak. An 18-year-old employee who was also trapped sustained non-life threatening injuries.

SAN JOSE, CA — The death of a construction company employee at downtown San Jose’s Silvery Towers site Thursday has triggered a state worker-safety probe into the development, which was already marred by allegations of underpayment of workers and project delays. The investigation by Cal OSHA began after the discovery that a person had fallen at the Silvery Towers site, where two residential towers are being built at 188 W. St. James Street near North San Pedro Street. The deceased individual, whose identity wasn’t immediately released, was on the payroll of Swinerton, a consultant on the construction project, which is being developed by China-based Full Power Properties, whose local offices are in Foster City.

Cookville, TN — A Cookeville man succumbed to injuries following an industrial accident late Wednesday morning. Algood Police Chief Dale Armour said Dewey Mayberry, 65, was performing typical job duties at M.E. Industries, Inc. in Algood when a highly pressurized tank went off and hit him in the chest. “They (the tanks) are about the size of a salad plate, so about six inches in diameter, but the metal is really thick,” Armour said. Mayberry was transported to Cookeville Regional Medical Center where he later died from the injuries. M.E. Industries Inc. is an refueling equipment company. According to the company website, it manufactures filters and filter separators for the U.S. military and commercial aviation customers. They also make custom filter housings and pressure vessels.

Volunteer firefighter dies while working at Lincoln County industrial plan
LINCOLN COUNTY, N.C. – A volunteer firefighter died Tuesday in an industrial accident in Lincoln County, officials said. Dewayne Adams served with the North Gaston Fire Department for the past five years. Officials said he was killed while working at American Tire Distributors. According to preliminary information provided by the Division, Adams was pinned between a wall and a tractor-trailer rig when the driver attempted to back up, unaware that Adams was standing behind the rig. Adams was transported to a nearby hospital and pronounced deceased.

Las Vegas, NV — The Clark County Coroner’s Office has identified an employee killed by a heavy piece of machinery early Thursday at a North Las Vegas architectural metal and metal-cutting business. North Las Vegas firefighters responded within five minutes of a call about the incident around 9 a.m. at Vegas Laser & Waterjet at 4188 N. Pecos Road, near Alexander Road, fire chief Joseph Calhoun said. When firefighters lifted the machinery, the man, 21-year-old Braden Liu, had already died, Calhoun said.
Railroad employee who died after being pinned under rail car wheel identified
Dallas, TX — The Dallas County medical examiner’s office identified the man as Julio Ariguznaga on Wednesday. The co-worker told police he had been in contact with Ariguznaga all night by radio, but he suddenly stopped responding. The co-worker then went looking for the victim. Dallas Fire-Rescue officials pronounced the man dead at the scene. A spokesman for Dallas, Garland & Northeastern Railroad Inc. said in a written statement that a conductor for the company died in a “tragic accident.”
